Office Policies:
We are currently scheduling new patients. Office hours are flexible, and appointments are available during the day, in the early evening, or on Saturday mornings. If you are a new patient, please arrive 10-15 minutes before your appointment. Appointments that are missed without at least 24 hours advanced notice are charged at the regular fee for the planned visit.
If you are a new patient, please bring your insurance card with you. If you have any other information that may be helpful - such as phone numbers for your benefits representative at work, the plan's case manager for claims, the claims processor for your employer group, or other important people to contact - please bring that information as well.
Our billing staff will submit claims to your insurance company for all services you receive, and you will be billed for any remaining balance. For reasons discussed here, insurance may not provide full coverage for services rendered. Co-pays are due at the time of service.
Our fee schedules will be provided upon request. We do not accept Medicare, Medicaid, or Badger Care. Patients covered under those plans can be seen in our clinic, but will be billed at our regular charges and will be responsible for the entire amount of the bill.
All of your medical records will be held in strict confidence. I do not use outside transcription services.
Payment options include cash or credit card. Personal checks are accepted only under certain situations that have been discussed in advance.
